The HealthFlex Limited Benefits Plan includes the HealthFlex Medical Discount Program that is not NOT health insurance. This plan provides discounts at certain healthcare providers for medical and ancillary services. This plan does not make payments directly to the providers of medical services. Members are required to pay the provider the discounted rate at the time of service. It is the members responsibility to confirm with Member Services that a provider is an active participant of the program prior to seeing that provider. Daily Hospital Confinement Benefit†: Pays the amount listed per day for up to 180 days if you have to be admitted and stay in a hospital for treatment of a covered accident or sickness.

Intensive Care Unit Benefit†: Pays the amount listed per day for up to 14 days if you are confined to an intensive care unit in a hospital for treatment of a covered accident or sickness.

Inpatient/Outpatient Surgical Benefit†: Pays benefits according to the surgical schedule as set forth in the description of coverage provided in your membership materials. The maximum amount payable for all surgical procedures performed in a calendar year for a covered person is $20,000. The maximum payment for each Surgery may be determined by multiplying the Point Value (2 to 200) of each procedure listed on the Surgical Schedule by the unit value for the plan you choose.

Ambulance Benefit†: Pays the benefit amount per transport for a maximum of 3 transports per person per year.

Accident Disability: Pays a monthly benefit for total disability resulting from a covered accident. Benefits are payable for up to 12 months and are subject to a 90 day waiting period.

Accident Medical / Dental Expense Benefit: Pays part of the expenses you are charged by a hospital, doctor, or certain other charges, up to a maximum of the amount listed if you are injured in a covered accident. Subject to a $100 deductible.

Accidental Death & Dismemberment Insurance: Pays the beneficiary up to the benefit amount listed for the member’s death or loss of certain body parts in a covered accident or a portion or that amount for the accidental death of a dependent, if this is a family membership, (50% for covered spouse, 20% for covered child(ren))

All members of the HealthFlex Limited Benefits Plan are automatically enrolled into the American Advantage Association (AAA) to be eligible to receive these benefits. The insurance benefits are underwritten by The United States Life Insurance Company in the City of New York, a member company of American International Group, Inc1, NAIC # 70106. United States Life is domiciled in the state of New York with a principal place of business of 830 Third Avenue NY, NY 10022-6565 and licensed in all states, plus DC, except PR. These benefits are under Group AD&D & Medical Care Insurance Policy (Form #G-19000) issued to AAA.

†Coverage is not provided for loss due to a pre-existing condition for 12 months from the Covered Person's effective membership date or for members over the age of 64. The Accident Disability coverage is not provided for members over the age of 69.
